PROXY FORM for the AGM:

If you are a Member but cannot attend the AGM you may appoint a Proxy to represent you at the meeting.  Complete the Proxy Form below.
Proxy forms must be received by 6:00pm on Monday 9 December (i.e. at least 24 hours before the meeting).  Proxy forms CANNOT be accepted at the meeting.
